Visualización ecográfica y diagnóstico prenatal de drenaje venoso pulmonar anómalo total infradiafragmático: a propósito de un caso

RESUMEN
El drenaje venoso pulmonar anómalo es un padecimiento raro
en sus formas parciales o totales, descritas según el número
de venas pulmonares que drenen en la aurícula izquierda. En
las formas totales se distinguen cuatro grupos, dependiendo
del lugar en el que drenen las venas pulmonares: supra, intra
o infra-cardiacas y, finalmente, una forma mixta menos frecuente.
Es una cardiopatía de difícil diagnóstico prenatal e,
incluso, postnatal, sobre todo en las formas parciales sin otras
anomalías. La asociación con cromosomopatías es baja; sin
embargo, está estrechamente vinculada con malformaciones
cardiacas complejas, sobre todo en el contexto de síndromes
heterotáxicos, casi siempre en las formas totales, lo que ensombrece
el pronóstico perinatal.
Se comunica el caso de un paciente con drenaje venoso pulmonar
anómalo total infracardiaco, en el contexto de una
cardiopatía compleja, asociada con canal auriculoventricular
completo y anomalía conotruncal, operado a las 48 horas de
vida con mala evolución posterior. Se describen las características
ecográficas y de angiorresonancia de esta anomalía y
su pronóstico y tratamiento perinatal.
