Tratamiento quirúrgico urgente de aneurismas de la arteria cerebral media asociados a hematomas intracerebrales

RESUMEN
Antecedentes: Los pacientes con hematomas intracraneales secundarios a la ruptura de una aneurisma necesitan de un
tratamiento urgente con evacuación del hematoma y clipaje del aneurisma.
Pacientes: Realizamos un estudio descriptivo retrospectivo
sobre el tratamiento quirúrgico y evolución de un grupo de 24 pacientes con hematoma intracerebral ocasionados por la
ruptura de un aneurisma de la arteria cerebral media.
Resultados: Todos los pacientes al ingreso tenían una puntuación entre 3
y 5 en la escala de la Federación Mundial de Sociedades Neuroquirúrgicas (WFNS). En 15 casos el hematoma era de localización
temporal y en nueve casos era silviano. El resultado final fue peor en los pacientes que debutaron con mayor deterioro neurológico,
anisocoria, hematomas mayores de 50 cm
3 de volumen y los que cursaban con obliteración de las cisternas del tronco cerebral.
Conclusiones: Los pacientes con hematoma intracerebral ocasionado por la ruptura de un aneurisma de la arteria cerebral
media se presentan con deterioro clínico rápido y necesitan de un tratamiento quirúrgico urgente.
