El continuum séptico: Comportamiento de indicadores inflamatorios

RESUMEN
Objetivo: Encontrar marcadores inflamatorios para aumentar la sensibilidad del continuum séptico.
Diseño: Estudio prospectivo durante un periodo de 15 meses.
Lugar: UCI polivalente de un hospital privado de la ciudad de México.
Pacientes: Un total de 136 pacientes (edad media de 59.5 ± 15.9 años) fueron incluidos.
Intervenciones: Ninguna.
Mediciones y principales resultados: Se registraron diariamente los siguientes datos: criterios de SIRS, sepsis, sepsis severa, choque séptico, balance ácido-base, ingreso de líquidos, balance de líquidos, cuenta de linfocitos y monocitos, velocidad de sedimentación, proteína C-reactiva, fibrinógeno y escalas de APACHE II, Bruselas y TISS-28. Se encontró: SIRS (n = 117), sepsis (n = 39), sepsis severa (n = 29), y choque séptico (n= 9). Las variables significantes fueron linfocitos (p 0.005), ingreso de líquidos (p 0.001), y las escalas de TISS-28 (p 0.001) y Bruselas (p 0.001).
Conclusión: Los marcadores inflamatorios son útiles para evaluar la evolución de la sepsis en los pacientes críticos.
