Mortalidad en la unidad de cuidados intensivos: evaluación con una escala pronóstica

RESUMEN
Objetivo: Evaluar el valor predictivo de la escala de APACHE II sobre la mortalidad en una UCI de México.
Diseño: Estudio prospectivo.
Lugar: UCI de un hospital de segundo nivel, Durango, México.
Pacientes: Un total de 211 pacientes fueron incluidos y evaluados durante las primeras 24 horas de estancia en la UCI.
Intervenciones: Ninguna.
Mediciones y resultados principales: De un total de 250 pacientes que ingresaron a la UCI se evaluaron 211. Se encontraron los siguientes datos en los sobrevivientes (n = 150) y no sobrevivientes (n = 52): edad media 60 ± 17.8 vs 55 ± 19.4 años (NS), temperatura 36.4 ± 0.8 vs 36.7 ± 1.2°C (p ‹ 0.05), frecuencia cardiaca 93 ± 265 vs 108 ± 26 latidos/min (p ‹ 0.05), creatinina sérica 1.32 ± 0.88 vs 2.25 ± 1.71 mg/dL (p ‹ 0.05) y escala de APACHE II 13.9 ± 9.1 vs 23.1 ± 8.6 puntos (p ‹ 0.05). La mortalidad predicha fue 37.7% y la observada 29.8% (NS).
Conclusión: La escala de APACHE II es útil para evaluar la mortalidad en una UCI de segundo nivel de México.
