Riesgo transfusional del uso de plasma femenino/masculino. Análisis y debate

RESUMEN
Se ha relacionado con la lesión pulmonar aguda por transfusiones (
transfusion-related acute lung injury [TRALI]), que es un síndrome caracterizado por dificultad respiratoria aguda después de la transfusión de componentes sanguíneos. La característica fisiopatológica de TRALI es un aumento de la permeabilidad microvascular pulmonar. Varios informes demuestran que la mayoría de los casos TRALI se precipitan por la transfusión de anticuerpos de donantes dirigidos contra antígenos humanos leucocitarios (HLA) o antígenos humanos de neutrófilos (HNA) expresada en la superficie de los neutrófilos de los del receptor. Esta interacción antígeno-anticuerpo se cree que causa la activación de los neutrófilos directamente y liberación de agentes citotóxicos, con el subsecuente daño endotelial y fuga capilar. Después de la transfusión de plasma se presenta un evento adverso importante no detectado. Algunos centros de sangre han limitado la recolección de plasma de las donantes mujeres que han tenido hijos, debido a su propensión a desarrollar anticuerpos anti-HLA después del embarazo.
