Anticuerpos tiroideos y manifestaciones extratiroideas en diferentes enfermedades tiroideas en población mexicana

RESUMEN
Objetivo: Evaluar la asociación de los anticuerpos tiroideos con manifestaciones clínicas en pacientes con enfermedad tiroidea autoinmune.
Sujetos: 430 pacientes con enfermedad de Graves, tiroiditis crónica de Hashimoto, tiroiditis subaguda y pacientes control.
Métodos: Se realizó historia clínica, se determinó anticuerpos contra receptor de tirotropina (TRAb): Inmunoglobulina inhibitoria ligada a tirotropina (TBII), inmunoglobulina estimulante de tirocito (TSAb), inmunoglobulina estimulante de crecimiento tiroideo (TGI), anticuerpo bloqueador de tirotropina (TBAb); anticuerpos antitiroglobulina (TgAb) y anticuerpos antiperoxidasa (TPOAb). Se evaluó la asociación entre las manifestaciones clínicas y los anticuerpos antitiroideos.
Resultados: Los TRAb fueron más frecuentes en hipertiroidismo comparado con hipotiroidismo (60.4 vs 32.5%, p 0.001) especialmente TBII (39.5 vs 14.9%, p 0.001), TGI (11.3 vs 4.4%, p=0.01) y TSAb (25.7 vs 12.4%, p=0.001), las manifestaciones clínicas asociadas a TBII fueron: diarrea OR 3.35 (95% IC, 1.80-6.23, p=0.001), pérdida de peso OR 2.37 (95% IC, 1.13-4.98, p 0.022), temblor OR 2.15 (95% IC 1.18-3.89, p 0.011), intolerancia al calor OR 2.27 (IC 95%, 1.14-4.52, p 0.019), taquicardia OR 2.38 (95% IC, 1.04-5.44 p 0.04).
Conclusión: Los anticuerpos antitiroideos están fuertemente asociados a manifestaciones extratiroideas, especialmente a diarrea, pérdida de peso, temblor e intolerancia al calor.
