Cistadenoma seroso del páncreas (“adenoma microquístico”). Informe de siete casos

RESUMEN
Las neoplasias quísticas del páncreas constituyen un grupo importante de tumores pues, aunque no son comunes, tienen una presentación clínica peculiar, un comportamiento relativamente benigno y alta probabilidad de tratamiento quirúrgico exitoso. La distinción entre quistes no neoplásicos y neoplásicos, así como la correcta clasificación entre estos últimos tiene gran importancia clínica. Las neoplasias quísticas más comunes del páncreas son los cistadenomas serosos (38%). Sin embargo, no estamos familiarizados con ellas, lo que impide una evaluación y un diagnóstico preoperatorio adecuados. Se trata de un estudio descriptivo y retrospectivo de un periodo de 10 años (1991-2000), en casos de autopsia y patología quirúrgica. Se informan siete casos de cistadenomas serosos del páncreas. Cinco de ellos corresponden a autopsias y dos a especímenes quirúrgicos. Todos se encontraron en adultos (100%), predominaron en el sexo femenino (71.4%), la mayor parte fueron hallazgos de autopsia (71.4%). Casi todos fueron lesiones únicas (71.4%), excepto dos casos asociados a síndrome de von Hippel-Lindau. En ninguno se efectuó el diagnóstico clínico. Uno de los aspectos más llamativos es la asociación de estas neoplasias con la enfermedad de von Hippel-Lindau. El tratamiento aconsejado es la resección quirúrgica total, que es curativa. El drenaje no es un tratamiento apropiado.
