Neumonía necrotizante hemorrágica y SARM-AC como causa emergente

RESUMEN
Staphylococcus aureus ha sido reconocido como una causa de neumonía adquirida en la comunidad. La neumonía por
S. aureus resistente a meticilina (SARM) se ha limitado a hospitales, principalmente a neumonía asociada a cuidados de la salud y a ventilación mecánica. En 1999, cuatro muertes pediátricas fueron reportadas secundarias a neumonía necrotizante por SARM adquirido en la comunidad (SARM-AC). Estas infecciones fueron causadas por cepas que difieren de las típicas cepas nosocomiales de acuerdo a sus patrones de sensibilidad a los antibióticos y a la electroforesis en campo pulsado (PFGE). En 2002, se encontró una asociación entre la presencia de leucocidina Panton-Valentine (PVL) una toxina estafilocócica asociada con necrosis de tejido y este síndrome descrito previamente de neumonía necrotizante hemorrágica aguda en los individuos sanos que presentan neumonía estafilocócica de la comunidad. Presentamos el caso de un paciente con neumonía necrotizante por SARM-AC, así como revisión de los orígenes, fisiopatogenia, manifestaciones clínicas y opciones de tratamiento para esta entidad emergente.
