Prevalencia de anticuerpos anti-VacA y anti-CagA en pacientes con dispepsia e infeccion por Helicobacter pylori y su asociación con úlcera péptica y neoplasia en población del Hospital General de Mexicali

RESUMEN
Antecedentes: la existencia de anticuerpos anti-CagA y anti-VacA en pacientes con infección por
Helicobacter pylori y dispepsia se ha correlacionado con neoplasia o enfermedad ulcerosa péptica.
Objetivo: determinar la prevalencia de anticuerpos anti-CagA y anti-VacA en pacientes con dispepsia, úlcera o neoplasia.
Material y método: estudio observacional, transversal y comparativo efectuado en pacientes de la consulta externa y hospitalización de Medicina Interna del Hospital General de Mexicali con dispepsia para procedimiento endoscópico. Se midieron los anticuerpos anti-CagA y anti-VacA en pacientes a quienes se realizó endoscopia del tubo digestivo alto y se correlacionó con enfermedad ulcerosa péptica y neoplasia. Se determinaron, la media y desviación estándar de los valores de IgG contra CagA y VacA y se compararon los grupos con pruebas paramétricas o no paramétricas de acuerdo con la distribución normal de las poblaciones.
Resultados: se incluyeron 41 pacientes con dispepsia y clo-test positivo. Tres pacientes resultaron con anti-VacA positivo y uno con anti-CagA positivo. Ninguno de los pacientes con títulos positivos de anticuerpos tuvo lesiones endoscópicas de enfermedad ulcerosa o neoplasia.
Conclusión: la probabilidad de tener úlcera gástrica, duodenal o neoplasia con títulos positivos de anticuerpos contra las proteínas VacA y CagA del
Helicobacter pylori en la población atendida en el Hospital General de Mexicali por dispepsia es prácticamente nula.
