Desarrollo de destreza quirúrgica endoscópica: ¿Cuándo ocurre la transición de novato a avanzado en un simulador físico? Estudio piloto

RESUMEN
Introducción: El presente estudio tuvo como objetivo determinar cuándo ocurre la transición de las destrezas laparoscópicas de novatos al nivel de avanzados con tareas en un simulador físico.
Material y métodos: Estudio cuasi-experimental con 10 residentes de cirugía pediátrica con ≤5 laparoscopías como cirujanos. Repitieron tareas del Sistema McGill (MISTELS), Southwestern y de García-Ruiz, hasta alcanzar una calificación de avanzados en 5 repeticiones consecutivas.
Resultados: El grupo requirió en general 12 ± 9 (2-60) repeticiones para alcanzar la calificación. En el nudo intracorpóreo requirieron 11 ± 4 (2-34) repeticiones, disminuyendo el tiempo de 155 a 61 seg (p=0.01); en el extracorpóreo, 16 ± 7 (4-29) con tiempos de 117 a 46 seg (p=0.002); y en el surgete continuo, 14 ± 6 (2-23) con tiempos de 559 a 180 seg (p=0.002).
Conclusión: Después de una media de 12 repeticiones, nuestros residentes desarrollaron destrezas que probablemente contribuirán a una mayor seguridad en sus primeras laparoscopías.
