Función cognitiva y factores cardiometabólicos en diabetes mellitus tipo 2

RESUMEN
Antecedentes: el deterioro cognitivo en pacientes con diabetes mellitus tipo 2 es un fenómeno bien reconocido; sin embargo, su causa es compleja debido a la diversidad de mecanismos fisiopatológicos potencialmente involucrados y a la alta prevalencia de comorbilidades, como: hipertensión arterial, obesidad y dislipidemia que, a su vez, pueden asociarse con alteraciones cognitivas y vasculares.
Material y método: revisión de la evidencia reciente acerca de los factores de riesgo relacionados con el decremento de la función cognitiva en la población diabética. Los factores revisados incluyen no sólo la glucemia y resistencia a la insulina sino también obesidad, hipertensión arterial y síndrome metabólico.
Resultados: el deterioro en la función cognitiva en pacientes con diabetes mellitus parece estar primordialmente relacionado con el control glucémico.
Conclusión: al momento, la evidencia sugiere un origen multifactorial del que destacan el descontrol crónico de la glucemia y como su marcador la HbA1c; sin embargo, para la detección oportuna se requiere atención integral de las comorbilidades y evaluaciones regulares de la función cognitiva.
