HISTORIA CLÍNICAbr>
Hombre de 34 años de edad, ambos padres con antecedentes de diabetes mellitus tipo 2. En los últimos años no cuenta con habitación permanente. Toxicomanías como tabaquismo desde los 14 años (IT 6 cajetillas/año), alcoholismo hasta la embriaguez cada tres meses. Consumo de marihuana y cocaína desde los 15 años; drogas intravenosas, compartiendo jeringas, al parecer suspendido a los 25 años, refiere ser heterosexual promiscuo.
Padecimiento actual: Lo inició seis meses antes con varios internamientos al hospital, con diarrea, tos, expectoración blanco-amarillenta escasa, fiebre de 38ºC, pérdida de peso de 17kg. Serología para VIH positiva, recibiendo antirretrovirales y antituberculosos por pocos días. Reingresó por tos en accesos, expectoración mucohialina escasa, fiebre y adenomegalias cervicales, se sospechó Tb ganglionar o linfonodonal y durante su estancia desarrolló moniliasis oral y un absceso en el 2º molar posterior derecho que ameritó extracción dental, así como ceftriaxona y clindamicina. Se le realizó biopsia de nodos linfáticos con diagnóstico de histoplasmosis. Recibió anfotericina B IV (dosis acumulada 750mg, administrada por 16 días). Fue dado de alta el 24 de enero de 2003 con persistencia de fiebre y adenomegalias cervicales, intratorácicas, con itraconazol oral 200mg cada 12h. Trimetoprima/sulfametoxazol 1 tableta cada 24h, HAIN 300mg cada 24h. Nistatina oral cada 6h.
