Excisión total de mesorrecto laparoscópica para el tratamiento de cáncer de recto en el Instituto Nacional de Ciencias Médicas y Nutrición Salvador Zubirán

RESUMEN
Introducción: La incidencia de cáncer de recto en occidente se ha incrementado en forma importante en los últimos 40 años. Los resultados oncológicos de la cirugía laparoscópica para el tratamiento de cáncer de recto aún son controversiales. La excisión total de mesorrecto constituye la técnica quirúrgica de elección en neoplasias malignas de tercio medio e inferior de recto. La excisión total de mesorrecto laparoscópica es segura, factible de realizar sin comprometer los resultados oncológicos.
Material y método: El diseño del estudio es observacional, descriptivo, prospectivo y retrospectivo.
Resultados: Pudieron corroborarse las ventajas que ofrece la cirugía laparoscópica para la evolución postoperatoria, sin encontrar repercusión en los parámetros oncológicos. La mediana de tiempo quirúrgico fue de 148 minutos, hemorragia operatoria de 320 mL. La estancia hospitalaria fue de 6 días y el inicio de vía oral a los 2 días. El 32% (9 pacientes) presentaron complicaciones postquirúrgicas.
Conclusión: El tratamiento quirúrgico laparoscópico de cáncer de recto es factible con una aceptable morbilidad y mortalidad; es técnicamente demandante y requiere un adecuado entrenamiento quirúrgico, así como el desarrollo de habilidades y destrezas quirúrgicas de mínima invasión.
