Carcinoma papilar primario de quiste del conducto tirogloso. Informe de un caso nuevo y revisión de la literatura

RESUMEN

Introducción: El carcinoma del quiste del conducto tirogloso es una entidad rara con una prevalencia aproximada de 1 %. El diagnóstico preoperatorio es poco frecuente y se realiza mediante biopsia por aspiración con aguja fina, aunque la sensibilidad es baja. El tratamiento óptimo es el procedimiento de Sistrunk, sin embargo, existe controversia en cuanto a si los pacientes se benefician con tiroidectomía total y el uso de terapia hormonal supresiva, ablación con radioyodo y radioterapia.
Caso clínico: Mujer de 32 años de edad quien acudió a consulta por tumor a nivel de la línea media del cuello diagnosticado como quiste del conducto tirogloso. La paciente fue sometida a resección mediante el procedimiento de Sistrunk sin complicaciones. El estudio histopatológico de la pieza demostró carcinoma papilar de tiroides el cual rebasaba la cápsula del quiste, por lo que se realizó tiroidectomía total, ablación con radioyodo y terapia hormonal supresiva. La evolución de la paciente fue adecuada.
Conclusiones: La evaluación inicial del carcinoma del quiste del conducto tirogloso incluye exploración minuciosa, ultrasonido y biopsia por aspiración con aguja fina. El tratamiento para este tipo de carcinomas debe ser selectivo y tiene que considerarse la tiroidectomía total en los pacientes con alto riesgo.
