La tromboembolia pulmonar (TEP) es un padecimiento que se presenta con frecuencia de manera silenciosa y se asocia a elevada morbilidad y mortalidad, ya que hasta en el 50% o más de los casos no se diagnostica y en un porcentaje aún mayor no se implementan las medidas profilácticas para evitarlo; tal vez una de las causas que pueden explicar lo anterior, es que en términos generales no se piensa en el problema por tanto no se previene, no se diagnostica y por ende no se trata; en estudios de necropsia se ha encontrado que hasta en 75% de los casos el diagnóstico no se hizo clínicamente. La mortalidad oscila entre 20 y 50%, aunque si se hace el diagnóstico en general es menor al 10%. La embolia pulmonar fatal es la causa de muerte hospitalaria prevenible más común, por lo que debería considerarse como un problema de salud grave. Por otra parte la trombosis venosa profunda (TVP) origen en el mayor número de casos del trombo que migra a los vasos pulmonares, no es prevenida; tal vez porque hemos minimizado el riesgo potencial que representa. En esta revisión analizaremos simultáneamente algunos de los aspectos relevantes con respecto a la trombosis venosa profunda y del tema que nos ocupa: la tromboembolia pulmonar.
