Polineuropatía del enfermo grave

RESUMEN
La polineuropatía del enfermo grave, es frecuente en la sepsis y el síndrome de disfunción orgánica múltiple. También ha sido descrita como una complicación del tratamiento con esteroides y/o bloqueadores musculares no despolarizantes, sin embargo, muchos autores sugieren que la polineuropatía del enfermo grave es una condición neurológica que es secundaria a múltiples factores. La forma más común de presentación es debilidad muscular y dificultad en el retiro del ventilador. Los estudios electrofisiológicos son la herramienta más útil para su diagnóstico; los hallazgos histopatológicos indican la presencia de polineuriopatía axonal motora sin cambios inflamatorios.
El propósito de esta publicación es revisar las causas principales, mecanismos fisiopatológicos propuestos, características clínicas y métodos diagnósticos y terapéuticos de la polineuropatía del enfermo grave, dado que en nuestro medio esta entidad es poco conocida y no diagnosticada en la mayoría la unidades de terapia intensiva.
