Heliox: A propósito de un caso de estado asmático refractario

RESUMEN

Objetivo: Reportar el uso de helio-oxígeno en el tratamiento de un paciente con estado asmático refractario.
Diseño: Reporte de un caso y revisión de la literatura.
Lugar: UCI de un hospital de tercer nivel de México.
Paciente: Un paciente de 55 años de edad con estado asmático refractario complicado con neumotórax, hipoxemia y acidosis respiratoria.
Intervenciones: Apoyo mecánico ventilatorio, instalación de una sonda pleural, administración de una mezcla de helio-oxígeno y uso de broncodilatadores.
Mediciones y resultados principales: Se efectuó monitoreo no invasivo, PEEP intrínseco gases arteriales a las 1, 12 y 24 horas después de iniciado el tratamiento con helio-oxígeno. Se observó disminución de la presión máxima de la vía aérea, de PEEP intrínseco de la PaCO2, así como aumento de la presión arterial media. También se corrigió la acidosis respiratoria.
Conclusión: El uso de la mezcla de helio-oxígeno es una buena alternativa terapéutica en pacientes con estado asmático.
