Tormenta eléctrica arrítmica en pacientes con cardioversor-desfibrilador

RESUMEN
Algunos pacientes con cardioversor-desfibrilador implantable reciben múltiples choques apropiados durante un corto tiempo, tormenta eléctrica arrítmica (arritmias ventriculares malignas que provocan tres o más intervenciones del dispositivo durante 24 horas). Es una complicación frecuente. Los objetivos de este estudio fueron determinar la incidencia, características, implicaciones pronósticas y opciones terapéuticas en la tormenta eléctrica. El estudio prospectivo de seis años incluyó 115 receptores del dispositivo, con un seguimiento de 510-2,100 días. La tormenta ocurrió por diversas causas en el 18.3% de los pacientes; a los 336 días (mediana) después de la implantación. Los pacientes con tormenta tuvieron 12 episodios arrítmicos y 9 choques (mediana) por tormenta. El 46.6% tuvo más de una, y fue la primera recidiva en el 60%. No hubo fallecidos. Se estudiaron dos grupos control (sin tormenta, con recidivas o no). La tormenta eléctrica fue responsable de más readmisiones hospitalarias, en comparación con los controles. El evento arrítmico terminó con una terapia combinada e individualizada, en todos se emplearon antiarrítmicos (en especial amiodarona y betabloqueadores). La estimulación antitaquicárdica resultó mejor tolerada que los choques. La tormenta eléctrica es un evento frecuente, serio e impredecible en pacientes con cardioversor-desfibrilador implantable, temprano o tardío después de la implantación, que puede manejarse con terapia combinada. No aumenta de manera independiente la mortalidad.
