Captopril-enalapril en el perioperatorio del paciente sometido a tiroidectomía. Ensayo clínico

RESUMEN
Antecedentes: La hipertensión arterial eleva de forma inaceptable el riesgo en los pacientes sometidos a anestesia y se hace necesario un tratamiento meticuloso antes de la operación con el objetivo de controlarla.
Objetivos: Evaluar las interacciones medicamentosas que se producen con el uso de captopril y enalapril en la aplicación de la anestesia general, en pacientes sometidos a tiroidectomía.
Material y métodos: Se estudiaron 106 pacientes sometidos a tiroidectomía, los cuales fueron divididos aleatoriamente en dos grupos de 53 sujetos cada uno. Al Grupo I se le administró captopril 25 mg/kg y a los del Grupo II enalapril 10 mg/kg; en ambos se utilizó la vía oral dos horas antes de iniciar la anestesia. Aplicamos anestesia general orotraqueal y se registró tensión arterial media de forma no invasiva, frecuencia cardíaca y saturación de la hemoglobina (SatHb), basal, inducción, transoperatorio y postoperatorio, así como efectos adversos más frecuentes.
Resultados: Se observó disminución de la tensión arterial media del captopril con relación al enalapril en el perioperatorio (p ‹ 0.05); el 39.6% de los pacientes presentaron hipotensión ligera.
Conclusiones: Captopril y enalapril pueden ser utilizados en el perioperatorio de los pacientes sometidos a tiroidectomía, a pesar de los efectos secundarios presentados.
