Tratamiento del prolapso rectal completo mediante la rectosigmoidectomía perineal.

RESUMEN
El prolapso rectal ocurre en los pacientes mayores, que no son a menudo candidatos
quirúrgicos debido a la presencia de comorbilidades. Por lo que la rectosigmoidectomía
perineal (Altemeier) es una opción segura y eficaz en el tratamiento del prolapso rectal.
Objetivo: Es conocer las complicaciones, mortalidad y recurrencia en la cirugía
colorrectal dentro de nuestro servicio de cirugía en el tratamiento el prolapso rectal.
Métodos: Entre 1995 y febrero de 2000, a 19 pacientes se les realizó la reparación
del prolapso rectal mediante el procedimiento de Altemeier. La edad promedio de los
pacientes fue de 71.4 años. Se recabaron los datos preoperatorios, intraoperatorios y
posoperatorios para su posterior análisis.
Resultados: 19 pacientes; 18 eran
femeninos y 1 era masculino, con un rango de edad de 32 a 84 años. El tiempo de evolución
de su sintomatología fue de 1 a 48 meses. De los 19 pacientes, 14 presentaban algún grado
de incontinencia anal preoperatoria. Las complicaciones ocurrieron en el 15.3% de los
pacientes. En 18 pacientes se logró una resolución objetiva del prolapso rectal.
Conclusión: El procedimiento de Altemeier es un procedimiento quirúrgico con
baja morbilidad y mortalidad en el tratamiento del prolapso rectal, por lo que está
indicado en pacientes de edad avanzada.
