Intento de suicidio en población pediátrica: abordaje integral en el área de urgencias

RESUMEN

El suicidio es un problema de salud pública, tanto a nivel mundial como en México. Se encuentra entre las principales cinco causas de mortalidad entre la población pediátrica y adulta joven. Este artículo tiene por objetivo dar a conocer al profesional de la salud, información actualizada sobre el abordaje integral del paciente pediátrico que ingresa a los servicios de urgencias por un intento de suicidio. Primero describimos datos relevantes sobre la epidemiología de la conducta suicida en niñas, niños y adolescentes y las principales presentaciones clínicas en las que se debe sospechar un intento de suicidio. Enseguida abordamos los factores de riesgo y posibles desencadenantes de conductas suicidas en NNA y algunos recursos que nos pueden ayudar a identificar y valorar a NNA con conductas suicidas. Finalmente discutimos sobre el abordaje integral y multidisciplinario en el servicio de urgencias.
Con esta revisión podemos concluir que el intento de suicidio es un problema que afecta a NNA y ha incrementado en los últimos años. El personal de salud requiere estar preparado para sospechar y detectar oportunamente a niñas, niños y adolescentes con intentos de suicidio, identificando y reconociendo las posibles presentaciones clínicas, los factores de riesgo y/o posibles factores desencadenante, así como, contar con herramientas que le permitan hacer una valoración del riesgo. Al ser multifactorial, esta problemática requiere atención multidisciplinaria, libre de estigma, centrada en el paciente e informada en el trauma. Cualquier intervención que se realice debe acompañarse de un plan de seguridad y seguimiento al momento del alta.
