Hipertensión pulmonar: valoración preoperatoria y cuidados perioperatorios en cirugía no cardiaca: actualización

RESUMEN
La hipertensión pulmonar es una complicación importante de distintas enfermedades
con fisiopatologías diversas; los individuos que la padecen requerirán algún
procedimiento quirúrgico urgente o electivo en algún momento de su vida. Esta
enfermedad se asocia con múltiples complicaciones en el periodo perioperatorio,
por lo que una estratificación de riesgo es apropiada para optimizar el tratamiento
específico y lograr un pronóstico favorable. En la práctica diaria, médicos internistas,
cardiólogos, intensivistas, anestesiólogos y personal afín se enfrentarán a esta
desafiante situación, en la que el conocimiento de los cuidados perioperatorios en
este grupo de pacientes es de suma importancia. Por ello, esta actualización destaca
la nueva estratificación clínica, a partir de la que se desprende el tratamiento
farmacológico adecuado, además del soporte circulatorio mecánico durante un
evento quirúrgico y en la unidad de cuidados intensivos, así como el reconocimiento
del daño multiorgánico secundario que amenazará la evolución clínica de
esta población vulnerable.
