Miocarditis por inhibidores del punto de control inmunitario: el futuro es hoy

RESUMEN
Antecedentes: La miocarditis por inhibidores del punto de control inmunitario es
una complicación potencialmente fatal en el tratamiento de pacientes con enfermedad
neoplásica avanzada, que cada vez se reporta más en todo el mundo debido a la
evolución indómita de la quimioterapia.
Objetivo: Describir la epidemiología, fisiopatología, detección oportuna, cuadro
clínico, diagnóstico y tratamiento de la miocarditis por inhibidores del punto de
control inmunitario.
Metodología: Búsqueda exhaustiva en diversas bases de datos: PubMed, Google
Scholar y ClinicalKey. Se seleccionaron artículos en inglés publicados de 2002 a 2023.
Se utilizaron términos de acuerdo con el esquema MeSH: inhibidores del punto de
control inmunitario, miocarditis, evento adverso relacionado con el sistema inmunitario.
Resultados: Se obtuvieron 899 artículos, de los que se seleccionaron 126, que
incluyen revisiones de la bibliografía destacadas, estudios observacionales, revisiones
sistemáticas, metanálisis, ensayos clínicos con distribución al azar, registros de pacientes
y guías de práctica clínica internacionales. El resto de los artículos se excluyó por no
cumplir los objetivos de la revisión.
Conclusiones: El diagnóstico oportuno de esta complicación inmunomediada
permitirá su tratamiento temprano.
