Infección diseminada por Kocuria spp.: reporte de caso y revisión de la literatura

RESUMEN
Presentamos el caso de un hombre de 42 años con antecedente de infección por vih, quien estaba en tratamiento
para criptococosis e infección por
Mycobacterium avium complex diseminadas, no obstante, cursó con deterioro
de su estado general asociado a disnea. Se identificó una nueva infección respiratoria complicada compatible con
empiema secundario a absceso pulmonar, durante toda la secuencia diagnóstica se encontraron tres diferentes
especies de
Kocuria spp. Se decidió realizar drenaje quirúrgico del empiema, además de iniciar cubrimiento antibiótico
con ampicilina sulbactam. Posteriormente presentó episodio convulsivo, se evidenció absceso cerebral por el
mismo microorganismo, se indicó tratamiento con moxifloxacino, además de manejo óptimo para su enfermedad
de base e infecciones oportunistas, pero el paciente falleció. Se discuten las características de este microorganismo,
los factores de riesgo que podrían precipitar una infección por dicha bacteria y sus diferentes presentaciones
clínicas, las dificultades en la identificación en el laboratorio y las alternativas terapéuticas.
