Systemic lupus erythematosus in males

ABSTRACT
Background: Systemic lupus erythematosus or lupus is a chronic inflammatory
disease that predominantly affects women of reproductive age and only 4% to 22%
of the patients are males.
Objective: To compare the clinical characteristics and outcome of lupus between
male and female patients.
Materials and Methods: A comparative cross-sectional study was carried out in a
second level hospital from 2010 to 2018. Men were matched 1:4 by age ± 5 years with
women. For the diagnosis of lupus, the new ACR/EULAR criteria of 2019 were used,
which include 7 clinical manifestations and 3 immunological criteria. The diagnosis
requires 10 or more points. The disease activity and the damage index were evaluated
only once by Mex-SLEDAI and the damage by SLICC/ACR.
Results: In total 127 patients were identified, from which 16 men and 64 women
were included. All had 10 or more points for diagnosis and the prevalence of positive
anti-DNA was 97%. Kidney disease was more likely in men at the onset of the disease
(11/16
vs 19/64; p = 0.006). End-stage renal disease developed in 5/16 men and 11/64
women (p = 0.18).
Conclusions: Men with lupus had similar characteristics to those of previous articles.
Men with lupus had more kidney damage than women.
