Disability due to balance disorders in patients older than 65 years

ABSTRACT
Background: Alterations in balance are a common complaint in the consultation,
affecting 20-30% of the general population. Approximately 20% of patients over 65
years have experienced some symptoms of dizziness or vertigo. The Dizziness Handicap
Inventory (DHI) is a useful instrument to document the consequences of balance and/
or vestibular disorders on the level of disability and functional impairment.
Objective: To assess the degree of disability in patients older than 65 years with a
history of balance disorder using the Dizziness Handicap Inventory (DHI) questionnaire.
Materials and Methods: Observational and analytical study done from February
2019 to February 2020, including patients in a tertiary hospital in Mexico
City, older than 65 years with a history of balance disorder. The DHI questionnaire
was applied.
Results: A total of 116 patients were reviewed, 30 were included. F/M incidence of
3.8:1, mean age 82 years. In 19 patients (63.3%) the disability impact was mild, in 6
(20%) moderate and in 5 (16.6%) severe. The functional sphere was the most affected,
polypharmacy predominated, and hypertension and major depressive disorder were
the most associated comorbidities.
Conclusions: The use of this type of tool is useful to assess balance alterations in
elderly patients since it not only measures the negative impact on their daily life, but
also evaluates the progress after a therapeutic behavior.
