ABSTRACT
It remains controversial the optimal timing of surgical intervention in the patient with chronic aortic regurgitation. There is no doubt about surgical intervention in the patient with sever aortic regurgitation and significant symptoms or functional limitation. However in the asymptomatic patient, the benefits of the intervention must be weighed against the risk of surgery, the expected hemodynamics after surgical intervention, the longevity of the prosthetic valve and the risks of anticoagulation. The real challenge is to identify the time point just before systolic dysfunction become irreversible in the patient with chronic asymptomatic aortic regurgitation.
