Painful ophthalmoplegia Tolosa-Hunt syndrome or Paraneoplasic syndrome

ABSTRACT
Painful ophthalmoplegia is characterized by orbital pain and ipsilateral oculomotor paralysis. Tolosa-Hunt syndrome (THS), caused by non-specific inflammation in the cavernous sinus or superior orbital fissure, is one of its possible causes. The THS is a diagnosis of exclusion therefore if there is a suspicious case of a patient who suffers from this syndrome, it is convenient to carry out the corresponding complementary examinations which include, among other studies the tests of brain image. This work refers to the case of a sixty-six year-old woman with these symptoms who was diagnosed with the Tolosa-Hunt syndrome and treated with steroids. She had a positive reaction to the treatment although she died 11 months later as a result of lung cancer. Taking into account this situation the author of this work asks herself whether there was a correct diagnosis of this case or not: Tolosa-Hunt syndrome or Paraneoplasic syndrome?REFERENCES
