Lactic acidosis associated to neoplasm

ABSTRACT
Objective. To report a case of acute lactic acidosis due to pancreatic neoplasm.
Design. Case report.
Setting. ICU of tertiary care hospital of México City.
Patient. A 62-yr-old woman with pancreatic carcinoma.
Interventions. Chemotherapy (gemcitabine, 1000 mg/m2/weak).
Measurements and main results. Abdominal CT scan, biopsy of pancreas (1996). A year later the patient was admitted to the ICU because she had severe abdominal pain, vomiting and tachipnea. Laboratory test revealed pH 6.8, PaO2 132 mmHg, PaCO2 13 mmHg and lactic acid 25.3 mg/dL. The treatment consisted in sodium bicarbonate infusion (3 mEq/h) during 11 hours. At the end of this time the laboratory tests showed pH 7.5, PaO2 97 mmHg, PaCO2 42 mmHg and lactic acid 6.6 mg/dL. She was discharged in good conditions three days later.
Conclusion. Lactic acidosis may be present in patients with cancer.
