ABSTRACT
Marjolin ulcer is the result of the malignant degeneration of a chronic wound or scar; its etiology is diverse and its evolution is fatal if the diagnosis is late. In order to draw attention to the existence of this entity and its possible manifestation in patients treated by the different specialties, given the importance of making an early diagnosis and timely treatment, the case of a 69-year-old woman with a history of injury is discussed. The patient presented with a burn injury of 20 years before admission, which left functional and aesthetic sequelae in the upper left limb. 15 years after having suffered the burn, an injury appears in the scar area which does not consolidate and evolves into an ulcer or respond to local or surgical treatment. The histopathological study reports squamous cell carcinoma in the lesion. It is evaluated by a multidisciplinary care team and Marjolin ulcer with axillary lymph node invasion is diagnosed, for which there was no other possibility than palliative treatment. Being a complication little known to non-specialized staff, the diagnosis was late. It is concluded that, before a chronic wound of any etiology, with three months of evolution and in which healing is not achieved, a biopsy should be performed in order to rule out its malignancy.REFERENCES
