De novo renal abscess open to surgical site as a complication of lumbar arthrodesis

ABSTRACT
Soft tissue and surgical site infections are an important complication of a surgical procedure. In spine surgery, they may be associated with deleterious effects, such as prolonged stay, increased postoperative pain or need for revision surgery. We present the case of a female patient operated on posterior lumbar arthrodesis and transpedicular L4-S1 instrumentation who was in the postoperative period with a de novo renal abscess open to the surgical site. The patient was managed multidisciplinary successfully, implementing empirical and then directed antibiotic treatment, single surgical washing, daily wound cleaning and finally vacuum assisted closure system and subsequent coping of the edges in the operating room. This is the first case of de novo renal abscess in the postoperative period of posterior lumbar arthrodesis not contiguous with aggravating opening to the surgical site in the mediated postoperative period. We present the established management, clinical evolution and favorable outcome for, lumbar surgery, renal function and the patient’s life.
