Compressive intrathoracic goiter: an atypical presentation

ABSTRACT
Background: Intrathoracic goiter is defined as growth of the thyroid gland that is
partially or totally in the mediastinum; patients may develop symptoms of obstruction
due to progressive compression of the trachea; the most common obstructive symptom
is dyspnea on exertion, which is present in 30% to 60% of cases.
Clinical case: A 60-year-old female patient who began suffering with tumors at
the neck level, facial edema and collateral venous network accompanied by dyspnea
on medium efforts. She referred to having to sleep with 3 pillows because she could
not be in supine position due to dyspnea; presented with stabbing neck pain without
irradiation in addition to difficulty swallowing solid food at times.
Conclusions: The treatment of choice for intrathoracic goiter is total resection of
the thyroid gland.
