Out of hospital deaths from myocardial infarction inLas Tunas: first sketches of a big problem

ABSTRACT
Introduction: The epidemiological, clinical and pathological characteristics of out-of-hospital death that occur suddenly and unexpectedly are poorly defined and have many limitations.
Objetives: To characterize some aspects of the out-of-hospital deaths due to acute myocardial infarction in Las Tunas province.
Methods: An observational, retrospective study of the out-of-hospital deceased patients with diagnosis of acute myocardial infarction was carried out in the period between January 2017 and December 2018. The universe of study was the 445 out-of-hospital deceased patients that had acute myocardial infarction as the cause of death during the study period in the province, as stated in the death certificate. The sample consisted of 125 cases in which an interview (verbal autopsy) could be made to close relatives who were present at the time of death, with prior informed consent. Descriptive statistics were used through percentage analysis for descriptive variables. In the case of age, the arithmetic mean with standard deviation was used.
Results: 62.9% of patients who died after hospitalization due to acute myocardial infarction were male. The average age was 72+ 9 years. Patients with low school level (74.2%) and of rural origin (58.9%) predominated. Only 21.3% of deceased patients underwent autopsies. 80.5% of the out-of-hospital deceased patients who underwent verbal autopsy were found to be hypertensive. 43.9% had a history of ischemic heart disease and / or heart failure. 53.6% of cases previously took angiotensin-converting enzyme inhibitors. 41.5% of deceased patients had precordial pain in the last hour. The time between the onset of symptoms and death was less than 1 hour in 46.3% of cases, while 73.2% died at home.
Conclusions: Out-of-hospital deaths due to heart attack were mostly male, elderly, of rural origin and low school level. There was a low percentage of necropsies.
