Use of the AST scale for alcohol withdrawal syndrome in Mexican population

ABSTRACT
Background: The alcohol withdrawal syndrome may occur in several degrees of
clinical severity, and specific treatment on it. There are many scales for staging the degree
of severity. Currently, the scale Clinical Institute Withdrawal Assessment for Alcohol –
reviewed (CIWA-Ar) is considered the gold standard to evaluate alcohol withdrawal.
Despite its usefulness, the CIWA-Ar is constituted by several items, implying more time
for the evaluator and broad subjectivity for the intervals of severity of a symptom. Anxiety,
Sweating and Tremor Scale (AST) is a more summarized and useful scale for symptom
assessment, in order to determine the severity of the alcohol withdrawal syndrome.
Objective: To evaluate the comparative performance of AST scale in Mexican patients
with alcohol withdrawal syndrome.
Material and Method: Cross-sectional and analytical study done in patients with
alcohol withdrawal syndrome from December 2017 to June 2018. A translation of the
AST scale from English into Spanish was performed. The reliability of the instrument
was evaluated.
Resultados: The AST scale was applied to 68 subjects with alcohol withdrawal syndrome,
Cronbach’s alpha = 0.852 and a Cohen’s kappa concordance = 0.618 (p = 0.0001). A
sensitivity of 84% and a specificity of 83.3% were obtained.
Conclusions: The AST scale is a useful instrument to determine the severity of the
alcohol withdrawal syndrome in Mexicans.
