Primary cystic neoplasms of the pancreas: Report on two patients, one with cystadenoma and the other with cystadenocarcinoma

ABSTRACT
Objective: Description of two patients with primary neoplasm of the pancreas, one with cystadenoma and the other with cystadenocarcinoma.
Setting: Second and third levels health care hospitals.
Description of cases: The patients were two women, 40 and 50 years of age; coursing with chronic abdominal pain, localized at the epigastrium and the right hypochondrium in the second women. The first presented a palpable tumor at the epigastrium, had lost 8 kg of weight, without jaundice or pain. Imaging studies, ultrasound and computed axial tomography of the abdomen, revealed in case one a cystic tumor located in the body and tail of the pancreas, in case two a cystic tumor in the tail of pancreas. Laboratory analyses reported no abnormalities. Both patients were subjected to laparotomy, resecting both pancreatic lesions.
The histopathology study revealed a mucinous cystadenoma of the pancreas for case one; and a mucinous cystadenocarcinoma of the pancreas for case two. This last patient developed a pancreatic fistula that was resolved on the 16th day after surgery with treatment through parenteral nutrition and octreotide (a somatostatine analogue).
Conclusion: Cystic lesions of the pancreas must be resected surgically, especially due to the risk of becoming malignant.
