Severe community pneumonia in children under 5 years. Some clinical and humoral aspects

ABSTRACT
Objective: to determine some clinical, epidemiological and humoral aspects related to severe pneumonia in the community in children under 5 years of age.Materials and Methods: an observational, descriptive and cross-sectional study was conducted in 112 patients with a diagnosis of severe pneumonia acquired in the community, attended by the Intensive Care Service of the Hospital Infantil Sur Docente of the province of Santiago de Cuba in the period from April 1, 2016 to April 1, 2018.
Results: children younger than 1 year predominated for 51.81% of cases, it was also this age group that developed complications more frequently, being the 2 most frequent acute respiratory failure and severe sepsis present in 24,99% and 21,42 % of the total cases respectively. The most common comorbidity/disease relationship found were anemia (42.89%), hypoproteinemia (35,71%) and previous use of antibiotics (14,28%). The acute phase reactants that showed the greatest utility were the leukocyte formula and the ESR. The most isolated causative agent was pneumococcus for 8%.
Conclusions: the age of less than one year was established as the most important risk marker for the appearance of complications in community-acquired severe pneumonias; hypoproteinemia as a form of qualitative malnutrition was also an incident factor in these patients. This last morbid state is recognized in patients with infectious diseases as it interferes with the transport of antibiotics that travel bound to the serum proteins.
