PFA-100: a platelet function test as a substitute for the bleeding time

ABSTRACT
Platelet Function Analyzer (PFA) is a new tool for the investigation of primary hemostasis.
It was introduced in the clinical laboratory since 1995 and is currently available as diagnostic tests.
The PFA is based on an instrument (PFA-100 and PFA-200), which simulates the blood flow, and two
cartridges containing membranes impregnated with collagen/epinephrine and collagen/ADP, which
simulate the tissue. From a technical point of view, the test is an In vitro equivalent to In vivo test
known as bleeding time. The PFA-100 can be used in different medical situations: as a screening test
of platelet dysfunction either congenital or acquired, as a diagnostic test in the case of aspirin and clopidogrel resistance, as a screening test in the case of follow-up of patients with von Willebrand
disease undergoing treatment with desmopressin, as quality control test in blood bank, in the case
of platelet transfusions; and as a legal-medical test in the differential diagnosis of hematological
diseases with mucocutaneous manifestations and child abuse, among many other indications. The
bleeding time has become an outdated test, it is not recommended in clinical practice by organizations
as the College of American Pathologists and the American Society for Clinical Pathology, and should
be removed from clinical laboratories services. There are enough evidence that the PFA-100, in addition
to being a noninvasive test, can replace the bleeding time with better analytical indicators, such
as sensitivity, specificity, positive and negative predictive value in the suspected of platelet function
disorders. In this module is presented to the medical community and clinical laboratory professionals a
new test for platelet function analysis with emphasis in the daily medical practice and technical issues
in the clinical laboratory when it is offer as a new test.
