Strange bodies in digestive tract associated to gastrocutaneous fistula. A case report

ABSTRACT
A 25 year old male patient is admitted with a history of multiple object ingestion, time is unknown, due to the patient’s vagrancy. The patient is also a 15 year drug addict, using different types of drugs. His current illness initiates after closed trauma approximately 15 days before admittance, referring intense upper abdominal pain, in addition a mesogastric orifice is observed, through which gastric material exists. Three days before admittance, he refers intense abdominal pain which alleviates with narcotics. At arrival, frank peritoneal irritation is observed. Urgent laboratory studies report leucocytosis of 19,000 and in simple abdominal X/rays, multiple radioopaque objects are observed. It was decided to perform exploratory laparotomy, locating a mesogastric orifice corresponding to a fistulous passage, anterior wall fascitis and stomach tract is used to extract the objects. A nail is located in the cecum reason why it was necessary to perform an appendicectomy. There were no complications but abdominal wall closure was deferred because extensive fascitis, treating openly and performing closure after 72 h. The patient evolved satisfactorily.REFERENCES
