Angioedema induced by metformin

ABSTRACT
Background: Angioedema is an edema that affects the deep layers of the skin; this
disease has prevalence between 20 to 25% of the general population.
Clinical case: A 40-year-old female patient with diabetes mellitus in treatment
with biguanides. She presented 10 days after the start of the drug a clinical picture
consisting of facial edema associated with pain in the left side of the face, ipsilateral
paresthesias, emetic and diarrhea episodes, she was managed with dexamethasone,
loratadine and withdrawal of the hypoglycaemic drug. Multiple studies were
performed that ruled out diseases, such as temporal artery arteritis, hereditary
angioedema and autonomic headache. For what it was considered angioedema
secondary to metformin.
Conclusion: The angioedema secondary to metformin has been shown in only
one case in the world, in a patient who presented a picture of anaphylaxis, it is a little
frequent adverse effect; differential diagnosis was made discarding other possibilities
and finally through Naranjo scale it was classified as a definitive adverse reaction.
