The pillars of cost-effectiveness: a practical guideline for new health technology cost-effective decision-making

ABSTRACT
Introduction: the use of health economics in health care decisions towards improved health is limited in Central America and the Caribbean. A health culture with an economic mindset that enhances access to new technologies is needed; one that increases the knowledge and application of economic evaluations among health stakeholders to make the most cost-effective decisions that improve health.
Objective: to develop a checklist for the medical and pharmaceutical industries aimed to make new technologies more affordable, available and cost-effective in resource-constrained settings.
Development: research was conducted to find new ways to make new drugs and technologies available to patients, healthcare providers, governments and society as a whole. The concept of Pillars of Cost-Effectiveness was coined based on three types of effectiveness ––clinical effectiveness, medical access effectiveness, and economic effectiveness. Research efforts resulted in the development of a practical guide to purchase new and innovative drugs in a cost-effective manner based on the concept of ―checklists‖ piloted by the aviation, medical and public health industries.
Conclusions: the Pillars of Cost-Effectiveness Concept and the New Health Technology Cost-Effectiveness Checklist may become the norm or standard to ensure the universal goals of patients, providers, policy makers and payers; that is: (1) purchase the best drug and technology quality at the lowest cost with reasonable access, and (2) enhance healthcare
partnerships and solutions towards improved individual and population health outcomes.
