ABSTRACT
Evidence-based medicine (EBM) is the conscientious, explicit and judicious use of the best current evidences when making decisions about the care of individual patients. It is not restricted to specific evidences, such as therapeutic results, but any clinical evidence generated by methodological research (random studies on cohorts, including a placebo one). EBM is not restricted to randomized trials and meta-analyses. For example, to find out about the accuracy of a diagnostic test, we need to find proper cross-sectional studies of patients in which such disorder could be taking place. Evidence-based transfusion medicine necessarily implies the application of these concepts in the following:
• Prevention of its risks
• Avoidance of its unjustified use
• Definition of its justifying concepts in agreement with all the areas involved in the transfusion process
• Raise in the healthcare costs
• Better knowledge of transfusion risks by general population
• Consequently, lawsuits for blood transfusion damage
Several studies have been made to have evidences that prove a better evolution for the patient. Nevertheless, there is also evidence that mortality at intensive care units is greater when red blood cells have been transfused.
