Therapeutical apheresis

ABSTRACT
The word apheresis is a term derived from the Greek «aphairesis», that means «to separate» or «to remove». Nowadays, apheresis procedures can be used with the aim of obtaining components for transfusion or with therapeutic aim (therapeutic apheresis). The American Society for Apheresis (ASFA) and the American Association of Blood Banks (AABB) have developed some guidelines of treatment for therapeutic apheresis; such guides are periodically modified according to medicine based on evidence. They have been classified in 4 categories, based on clinical response and the patient’s laboratory tests: category I, treatment accepted; category II, treatment with enough evidences of being effective in combination with other treatments; category III, controverted evidences of its effectiveness, and category IV, its effectiveness is not known and controlled studies are required. It is necessary to take into account the following when making an apheresis procedure: a) clinical history of the patient, b) letter of informed consent, c) vascular access, and d) evaluation of replacement liquids and consideration of adverse side effects to the access, the procedure and the anticoagulant.
