Left atrial affectation in non complicated high blood pressure patients

ABSTRACT
Introduction: There is little knowledge about the precocious changes of the left atrium (LA) in patient with arterial hypertension and there is the presumption that morphological or hemodynamic changes could occur on it in these patients.Objective: To identify the echocardiographic alterations in the left atrium of hypertensive patients without clinical complications.
Method: A descriptive study that included 62 hypertensive patients without complications was designed, a control group paired by age and sex with 62 individuals was conformed, transthoracic echocardiographic study was performed to all patients and controls to evaluate: the indexed volume of LA, the relative thickness of the wall and relaxation pattern of the left ventricle, the relationship E/e', e' wave velocity at level of the mitral ring and the isovolumetric relaxation time (IVRT). It was registered the evolution time of the hypertension and the risk factors (RF). It was used the test comparison of proportions, with the statistic χ2 and Student t for comparison of mean, it was, considered as statistical significant level a p value < 0,05.
Results: The mean age of the hypertensive patients was of 65 ± 10 years-old and for the control group 64 ± 10 years-old, the women were 67,8% in the study group and 62,9% in the control group. The evolution time was between five and 10 years in most of the patients (46,8%) and only 9,7% surpassed the 10 years of evolution, none of the RF presented association with the changes in the LA, the most frequent RF was obesity (22,6%). The mean of IVRT, lateral e' velocity and the indexed volume of the LA, were significantly higher in hypertensive patients.
Conclusions: The indexed volume of LA, the IVRT and the lateral wave e' velocity were increased in hypertensive patients in comparison with the non-hypertensive population.
