Carcinoma ex-spiradenocilindroma of scalp. Case report

ABSTRACT
Introduction: Spiradenocarcinoma, represents only 0.005% of all skin tumors, is rare and aggressive, predominates in older people, without predilection for gender, is rare in the scalp and eyelid, reason for the presentation of the case. Clinical case: Female patient of 66 years without comorbidities, in July 2014 due to scalp tumor, biopsy reporting spiradenocilindroma, with deep bed and positive margins, presented recurrence in surgical site with surgical resection plus reconstruction in May 2015, with histopathological report of ex-spirodenocilindroma carcinoma, negative angioinvasion and neutrophism, positive deep bed at 3 months postoperatively with right neck metastasis, in July 2015 dissection of the right neck and then external radiotherapy of intensity modulated escalating dose to 66Gy, ending in October 2015. Conclusion: The malignant transformation of the spiradenocilindromas is rare, they present local recurrence and distant metastasis, the mortality of 39% in the absence of treatment; Wide resection is recommended as well as adjuvant radiotherapy to reduce the risk of recurrence, however there are few cases reported in the literature, and there is no established treatment.REFERENCES
