Osteoarticular tuberculosis of the ankle, arthrodesis with transcalcaneal nail Case report

ABSTRACT
Osteoarticular TB rates range from 7 to 25% with the spine, hip and knee accounting for 80% of the total number of cases, followed by the tarsus, carpus and ankle.
Objective: To report a case of tuberculosis of the ankle and the treatment options.
Clinical case: 49-year-old female seen at the Ministry of Health General Hospital, Mexico City. In June 2000 she started having right forefoot and ankle pain of 5/10 intensity, without an apparent cause, that worsened with activity to 8/10 and decreased with rest and NSAIDs. Baseline x-rays were normal. In august 2001 the ankle pain persisted and did not subside after two infiltrations; she could not walk and had swelling. The biopsy taken on May 2002 reported granulomatous synovitis, she was found to be PPD-positive, and underwent anti-TB treatment for one year. During treatment she reported 10/10 pain and swelling of the right ankle. The x-rays showed a decreased joint space, osteoporosis, irregular joint areas, which were proven by MRI. She underwent surgery to place a transcalcaneal nail.
Results: The patient did well, arthrodesis occurred at 8 weeks with proper function.
