Posterior occipitocervical stabilization

ABSTRACT
Using a simple autologous bone graft was the beginning of the construction of an occipitocervical fusion mass, because of high rates in patients of pseudoartrosis, deciding to link the use of the graft by subjection with wires in order to hold it on the cranium to cervical level. This technique was not enough; it was observed cervical spine telescopements in the foramen magnum. Harrington and Luque started using simple rods to increase safety and efficiency. During 70’s and 80’s the use of plates and screws in lumbar spine region were stabilized. Extrapolating these principles, the use of plates and screws in occipitocervical region initiated, too. Originally, screws in the cranium were popular using the bilateral or bicortical position, but the reports indicated a high rate of weakening. So, it was proposed and experiment with medial screws held to internal occipital crest. Degenerative, trauma, tumor, and congenital pathologies which cause instability are tributaries to posterior stabilizations that represent a broad-spectrum and multidisciplinary segment, so particular of the spine cord. From a neurosurgical point of view, the approaches decompressed of posterior cranium base are located in a continuous evolution process; this is not necessarily happen in orthopedic concepts which have as an objective to control the stability and function of the occipitocervical union.
