Basal cell adenoma: presentation of five cases of a rare tumor of salivary glands

ABSTRACT
Introduction. Basal cell adenoma is an infrequent epithelial neoplasm of salivary glands of monomorphic histological appearance
with predominance of “basaloid” cells.
Objective. Classify 5 cases of basal cell adenoma according to their histological patterns and
perform the differential diagnosis with other salivary tumors of different prognosis.
Methods. The histopathological study was made (H/E,
Masson, Ki 67, CK AE1-AE3 and sensitive markers of myogenic differentiation -p63, alpha smooth muscle actin) of 5 BCA located in:
parotid (n = 4) and upper lip (n = 1).
Results. The most common histologic pattern was solid. However, tubular and trabecular and cystic
areas were observed within the solid component. In one case areas of membranous pattern were found and in another case the pattern
was exclusively tubular. No cellular atypia or mitosis was observed. With p63 and alpha smooth muscle actin the myoepithelial component
was observed. The labeling with AE1-AE3 was positive in the epitheliocytes. Ki 67 was negative.
Conclusions. Of the five cases reported
in the present study, the location of one of them in the upper lip is considered an atypical manifestation of a tumor almost exclusively
parotid. Their low cell proliferation allows them to be differentiated from their malignant counterpart, basal cell adenocarcinoma. In the
differential diagnosis, we also consider pleomorphic adenoma, canalicular adenoma, adenoid cystic carcinoma and basal cell squamous
cell carcinoma. Basal cell adenoma is a rare tumor of good prognosis that must be differentiated from benign and malignant tumors of
equal location. Immunohistochemistry indicates that epitheliocytes and myoepithelocytes coexist.
