Primum non nocere: Osmotic demyelination syndrome

ABSTRACT
I present the case of a 47-year-old woman with a diagnosis of myoepithelial carcinoma in the right neck who develops osmotic demyelination syndrome during her hospital stay due to a rapid hyponatremia correction. Brain magnetic resonance imaging was performed four days after the sudden onset of neurological deterioration in which pontine restriction in diffusion sequence was identified without alterations associated in FLAIR or T2 sequences, in addition, bilateral extrapontine demyelination signs were seen. The pontine diffusion restriction has been mentioned as an early finding in the recognition of this life-threatening syndrome.
