Vestibular paroxysmia and hemifacial spasm treated with microvascular decompression through retrolabyrinthine presigmoid approach with endoscopic support

ABSTRACT
We report the case of a 52-year-old patient with a nine years bilateral pulsatile tinnitus, predominantly left, recurrent episodes of vertigo and dizziness associated with stress, and three episodes of ipsilateral hemifacial spasm. She was diagnosed with vestibular paroxysmia from microvascular compression of the VII and VIII left cranial nerves who experienced a remarkable improvement after microvascular decompression through a retrolabyrinthine, presigmoid approach with endoscopic support. The described approach allows auditory preservation and, with the support of endoscopy, the neurovascular package is visualized in detail in the cerebellopontine angle.
