ABSTRACT
We report a case of descending thoracic aortic aneurysm resolved in a hybrid manner with open and endovascular management. A 72-year-old female with no history of smoking or chronic-degenerative diseases, comes for medical help due to chest pain, when performing a chest radiograph, the widening of the mediastinum is evidenced, indicating an angiotomography showing an aneurysm of the thoracic aorta descending, it was decided to perform a carotid-subclavian bypass because the aneurysm is located at the birth of the left subclavian artery, after which a stent was deployed in the descending thoracic aorta. The endovascular exclusion of thoracic artery aneurysms involves in some cases the coverage of the birth of the left subclavian artery to ensure a proximal anchoring site of the stent; at present we find low quality evidence that supports the benefit of revascularizing all the patients in whom the birth of the left subclavian artery should be covered, so that other surgeons support revascularization in selected patients, higher quality evidence is needed to strengthen the recommendations.
